Contest Giveaway Rules
- Complete and submit the form below completely with contact information
- In the Comment Field add in which of mat you'd prefer (if any). We will try our best to honor requests, but it depends upon availability at the time of the prize draw so we do not guarantee availability
- Must be present at Adepticon and able to pick up the prize on Sunday, March 26th. Prize cannot be given to a designee or surrogate. Must show valid ID that matches entry form when picking up the prize
- Winner will be selected at random at 10:00 AM on Sunday, March 26th and will be contacted by both e-mail and text message from their entry form and name announced on Facebook \ Twitter.
- Sign up to the contest agrees to be contacted and have their name announced, picture taken and used on social media and our webstore
- If winner is not able to pick up prize and notifies our team, we will pick an alternate winner and notify them
- Completion of the form indicates agreement to sign up for our Newsletter as well
